So we all know that Miko was based on a real life character Prince Shotoku, we all know that she changed her body to a more modern form, so is she technically a transwoman?
For me the answer is probably not, and Miko could simply have always been female, but there are some reasons make me think otherwise:
1: In Sanae's good ending, quote: "At dinner, Sanae's confused by how a old bearded guy on an old 10,000¥ bill and one of the hermits she met are supposedly the same person. It notes how she didn't care for science or history that she'd try reading up some and that being in Gensokyo she has no access to current rumors from the outside world, one of which was that Prince Shoutoku was a sham and that Sanae doesn't realize that's the cause of the incident." So does that implied Miko used to be a bearded man? I can't explain this part at all
2: Some says that Futo didn't realize Miko at first (this is probably uncannon because I can't find it anywhere in the game) so does that mean because she changed her gender so no one realized her?
So that's some reasons that I can't explained if she was a woman all along, hopefully the comments will help. But here's some reasons I think she was a woman
1: Miko is based on Prince Shotoku, not necessarily means she has to be exactly the same person, the same guy, so she could be whatever gender ZUN wants her to be
2: When a Shikaisen "reborn" they will normally come back with an appearance as close as them as possible, not only that makes everyone recognize them, but also because they can feel familiar with their newer body. So she could always have been a woman
