You can do it easily with a metadata editor.

Download mp3tag, go to the file manually and right click it to open it quickly with the mp3tag menu option or just open mp3tag and use the filebrowser in there. The filebrowser can take a while if you have a shitload of files in a folder. Click on the file you want so it's highlighted in blue, and then right click menu 'add cover' on the CD image on bottom left. Might have to refresh the directory for it to show up if it doesn't initially.

For .MKV files you need to rename your image that you're going to use to 'cover.jpg'
