While Cookie is generally a "???" in most casual touhou fan circles outside of JP, or just gets labeled as doxxing or trolling, I think you're right on the ball in regards to the mentality of anons and JP-speaking Western touhou fans. INMU has gotten similar from people either mistaking it for Wrestling Series/Gachimuchi, or just not knowing it to a be a fad.
It's not like people outside of Japan don't get it, but there is so much context that's lost just by not understanding Japanese language and web culture in general, yet alone the 2/5ch/nnd sphere of things, that just being exposed to a byproduct of it isn't going to be that interesting. You have to go out of your way to understand it.
Anons on imageboards tend to be the kind of people that have the closest inherent sense of that kind of culture over other communities. Especially the idea of intentionally contributing time and talent into a low-merit/taboo topic for no other reason than personal entertainment and expression among an asocial audience.
