That's from the fourth KnK movie/novel, IIRC. Touko sees Ryougi in the hospital bed mentally preparing herself for gouging her own eyes out because she's horrified with what the mystic eyes are making her see, but Touko makes her reconsider after telling her that even if she physically destroyed her eyes she would "still see what she was supposed to see".
