Primary position:
1 - Left ring
2 - Left middle
3 - Left index
4 - Right index
5 - Right thumb
6 - Right middle
7 - Right ring

Scratching (right thumb isn't busy with 5):
1 - Left thumb
2 - Left index
3 - Right thumb

Scratching (right thumb is busy with 5):
1 - Left thumb
2 - Left middle
3 - Left index

And then a lot of other very situational hand positions (right hand on 2, left hand on 4, whatever) that I rarely use.
