Yasu is only transgender if you believe Yasu was born as a boy, and even then it is only a product of how Yasu was raised that lead to a female identity. It can barely even be considered 'transgender', as Yasu did not even know her original gender until told about it by Nanjo, and has lived their life believing she was born a woman, whether that is true or not.
Lion has had a good upbringing, and the only insecurity Lion feels in that sense is that people can't actually tell whether Lion's a boy or a girl, hence why pronouns are awkwardly avoided when referring to Lion in the translation, because it is a lot easier to talk about someone without using pronouns in the original language of Japanese than it is in English. It's nothing to do with transgender identities, but simply insecurity that no one can tell easily whether Lion is a boy or a girl.
