Well my rules are pretty simple, I don't think I am lenient. Here:

Card contains Kanji?
Fail if I don't know both the meaning and reading. No exceptions.

Card doesn't contain Kanji?
Fail if I don't know the meaning and the interval is greater than one month.
If the interval is shorter than one month I will close my eyes and see if I understand the word in the example sentence (training to recognize in context). If that works I grade the card hard. Obviously this only works in Core6k with voiced example sentences, I fail those cards in my mining deck. This is mainly a result of me being frustrated at Kana cards, honestly Kanji are a huge memory crutch.

Meaning is easy because I already know the Kanji, like you said.
Reading is often easy because I can infer readings from other Kanji I know.
The main challenge is being disciplined and learning for that long, obviously good reviewing speed is a prerequisite.
