Summary from said book:

Given that a monsterized woman won't become an extremist unless she has the proper aptitude (namely that she comes to agree with their vision and desires to achieve it their way), the Extremists have instituted what they call an Organization of Breaking, which trains women in their philosophy before they are monsterized (the methods of training are not specified but are noted to harshly and thoroughly seed the Extremist's philosophy into the minds of the women).
