Are drugs that big a problem in japan? I realize I should try to learn form more then manga but why make this giant PSA against drugs if no one does them over there? Also what are they saying?

So I've learned that pretty much no one does drugs in japan. So should I believe my bosozoku manga though and think 3 out of every 5 people smoke cigarettes?
