Satoshi was desperately trying to convince himself that Satoko wasn't to blame even though she was the one picking up fights with her family, but deep down he knew it was the truth and wanted to violently lash out at her, so he told Rena about it, who then told Shion alongside the fact that Satoshi was about to disappear because of Oyashiro-sama's curse. Satoko herself would later also admit to this fault and is the biggest reason why she acts the way she does when Teppei arrives in Hinamizawa.

I think she was referring to Satoko causing him to be hurt by his aunt by making her mad over and over and him having to step in.

Fun fact, Satoshi wanted to hurt Satoko too for that same reason.
