That's a fanart of her, since they've never shown her without the mask (or even said if there's anything under it). This one is closer to her in-game design.

Ignore the fanbase. That's what I do at least, and the game is much more enjoyable for it. But yeah, she's probably my favorite champion design-wise in a long time. Her art, voice, and music all make her seem like she came from a Studio Ghibli movie or something.
