Well, since this board is filled with perv weaboos that can't think with anything else than their dick and are interested in tōhō just for trivial and sexual things and no one answers seriously the question, I'll do it;

Everything started with a fanmade comic named "Cold Wind but Warm Winter" from Happy Flame Time.

At the page 29, there is a small scene where they make Sanae drink and she gets drunk and says weird things — including saying she's a good girl and asking Kanako and Suwako to tell her that she is, most likely because she felt guilty to have let the two others drawn her to drink and wanted to be comforted.

That's all. She never actually acted in a single lewd way, even less in the official background.
Like most of the fan jokes (like sakuya's pads), it often starts from something insignificantly small, totally unrelated and meaningless, without any actual relation with the actual character's background.

Just because she says (and wants people to say) that "she's a good girl", even though she was talking in the literal sense, the fandom took it in the other sense and started to make jokes about how Sanae is a "naughty girl' in questionable contents.
