To partially answer both questions. There's the option of paying to get streaming access HD releases, including a lot of titles that only had DVD physical releases.
Not all are easy to use though, like DMM apparently requires you to use a VPN. There's that sokmil.com/ site someone mentioned a few threads back, but not sure if they get all titles.
SpiceVisual also have their own digital distribution site spice-tv.jp, were u can get FullHD versions of their DVD's. And you can apparently get early access to releases through the site too.

Someone gave a explanation of how to use DMM for digital releases.
>You need a DMM account and a Japanese VPN to buy them. I recommend using Softether/VPNGate.
>DMM uses a points system and you can charge points from your credit card/Paypal. (1 yen = 1 point). You can then use these points to buy gravure videos.
>The part where the VPN is required is when you actually go to the video page and try to pay for it using the points that you have. (DMM tries to restrict non-Japanese IPs from buying anything for whatever reason). You'll also need the VPN in order to download the video from their servers after you've bought it.
>Just make sure you're not connected to the VPN when charging points from your credit card/Paypal. Paypal has a strict no VPN policy and credit card transfers over VPN can be pretty risky as well.
