Despite excessively graphic rumours, it seems all Koakuma really does as Patchy's familiar is
1) putting books back in their places.
2) keeping the library tidy and clean (why Sakuya doesn't take care of this is unknown).

It's that tender and fairly innocent, yet incredibly alluring way Koakuma traces Sakuya's not quite 100% genuine chest. It invokes more emotion than it should. Among other things it invokes a slight envy and a reminder my two friends aren't getting a lot of love at present.
