Curses.

Each person has a curse, which is like a prohibition or rule (i.e.: live as a girl), and every one has to live by keeping it secret. If they fail at it, the curse comes to claim their life. Because life is kinda shitty, these cursed people gather together (hence the title, birds of a same feather flock together) to help each other cope though the everyday disadvantages of their conditions and maybe find a way to lift the curses.
