Just found about it in the game information. Everything you experienced there has an explanation:
Every character has a change to perform a preemptive attack, a combo attack or a counterattack. Apparently they're not affected by any stat, aside from the direct modifiers like
>counter rate up 30%
I think some weapons have these modifiers too. Especially considering that the third option on equipment is a randomized one.

It took me only 3 runs to get all the materials for upgrading a 3 star to a 4 star. I'm glad we only need to run one type of the quest for the evolve mats.
