All they knew is that Keiichi is acting weird and scared(like Satoshi when he disappeared in last year's curse), their plan is to continue the fun days of club games/pranks to try keeping things normal. We know that if Mion and Rena didn't hide things from Keiichi initially(they didn't want to scare him) he might've gotten past his paranoia about the village. At some point, he was too far gone and telling him would only add fuel to the fire. The last scene with the marker could've happened much faster from their perspective, Keiichi freaks out and grabs that bat, and bonks them on the head.

The reason the doll is mostly irrelevant is that it's just a vehicle to trigger Shion's issues through Mion. When Mion actually gets the doll in Minagoroshi, it doesn't matter for Shion anyway because Shion has gotten past her jealousy towards Satoko/Mion, and is even in her big sister role at that point. So the second doll scene mostly serves to help give Rika hope that things can change for the better. As in, make Rika actually bother trying to fix things.
