Does gaijin really mean not japanese, or just outsider in general?

For example, would i be weird for people say gaijin to a japanese person if he's not in japan? e.g. talking to a japanese person in america.

Similarly, would this be functionally the same in a fantasy novel, where outsiders would still be referred to as gaijin?

naturally just being a conjugation of outside and person, is it unique distinct to the land of japan or is it a general term for outsiders from a native stand point?
