She is drawn with breasts and a smaller third eye here and three other official works. ZUN agreed it looks better, don't bother him about it.

It doesn't really matter though because Okuu looks good with any chest size anyway. I'd tenderly love her with or without breasts.

Yeah, that kind of sucked. But at least she's playable. She was probably the very last character he tried to shoehorn in. It always felt like ZUN tried to somehow think of a way to work her into the story beyond SA, what with her several cameo appearances and all (two of which looked like foreshadowing) but he just couldn't figure out how.
