A bunch of the drama came from Yasu being raised to believe they were the opposite sex of what they really were. A guy without a penis could be led to believe they were a girl until puberty proves otherwise, but a girl isn't going to be tricked into thinking she's a normal boy. There's also the issue of their breasts being fake. It's never portrayed as them simply being flat, but rather that they physically cannot grow boobs.
