The first chapter starts with the entire town going crazy and Keitaro scratching his neck and bleeding to death, so Hinamizawa syndrome still seems to exist.

We'll probably know more about the past when they mention the Oyashirosama curse, since that should make it clearer what was the "curse" in this world.
