Huh, accidentally upped page 2 first. Here's page one.

I couldn't find the original recording, so here's a quick render using the crippled Pianoteq 3 trial (with missing notes). This midi is an old version that has the fadeout ending, though the "ending" on the score is just something I made up anyhow.
