This is ZUN we're talking about here. He probably a) didn't notice it, b) was too drunk to notice, or c) originally designed her as a human but thought her design was too boring so he added the cat ears at the last second.

I'm just as bad as ZUN though, I didn't notice her four ears until someone pointed it out. Ever since that day I can no longer fap to her because I'm always reminded of her flawed design. Hopefully she's using all four of her ears to hear this request: be a good (necrophile) cat girl and cut off your cat ears and become a human. I bet you furfags didn't see that one coming.
