Mugenkan is meant to be at the very least in the borderspace between Makai and Gensokyo, the lake of blood appears to just be a shortcut.
Seeing that it's a hop, skip and a jump away from Makai, and the LoB is in a very difficult place to get to conventionally, I would think it more likely that Kurumi came there from Mugenkan, probably first from Makai.
Besides, anything else would destroy my headcanon of Elis sometimes visiting her distant relative, which is too cute for me to not defend.

Although I would agree that Kurumi would talk with a rough country bumpkin accent. The gap moe between her outdated and provincial speech and the little blonde gremlin is also very cute.
