I dunno, to me the scene really feels like Ooishi is putting it together right then, and had suspicions because of the notebook when Shion switches the names around. It's definitely possible he had suspicions earlier but at the very least he only confirms it here

You know, I'm still curious how Ooishi finds out about the Shmion tattoo through an informant.
I guess the switch is less of a secret than we though.

I just think it deserves a little credit when Shion had everyone fooled and no one even found out what truly happened or could guess that the twins switched. They ARE identical twins but they act very differently, and Mion acting like Shion is a much weaker disguise than the other way around.
Well, Ooishi may have figured some of this out after everything went down and people were already dead. Yes, she had a lot of resources available by pretending to be Mion, but she did use them decently enough, Mion has had that position during all the curse shit and was always too passive in comparison.

Shion was never going to find out the truth about Satoshi, because no one she could even suspect knew anything anyway. All she finds out is that Oryou and the Sonozakis bluff and take credit for shit, and that Mion didn't even know until after Satoshi was gone. Unless she blamed Irie or something and targeted him, but she had no reason to believe he was anything but an ally to Satoshi, and even then the Yamainu would protect Irie.
