Touhou games aren't even close to the toughest shmups out there. Most players of the genre consider CAVE and Raizing games to be quite a bit more difficult, especially in the titles with second loops. I think some of the Lunatic difficulties can get quite tough there, especially with SA and DDC, or the extra stages.
