Out of curiosity, how did those of you here who are fluent or advanced learn Japanese?

I know the DJT guide lists quite a few different approaches, but I'm curious as to which ones successful learners actually followed. Did you use Anki? Did you go any more in-depth into grammar study than just reading through Tae Kim's guide? Did you do RTK or anything similar? Is reaching fluency really a straight-forward as just spending a few years reading VNs and so forth while looking up any words you don't know?
