Karma. You have a balance of it and must atone for sins. Doing things such as killing your parents, genocide, causing a schism, harming a Buddha, and killing enlightened people are basically irredeemable sins and you go to worst hell. Technically you can work it off like you can with other sins and bad karma, but the heat death of the universe is more likely to happen before then (3.4 quintillion years and we are 14 billion from universe start).
Still, Buddhist hells at least aren't permanent and eternal damnation like Christianity's hell is. There is a way out for everyone, it just takes time and spending your lives living virtuously.
In Touhou it's more or less the same except there is no way out because the faggot divas decided pools closed. At least the Netherworld is peaceful and easy, and even if you do go to hell there's a chance you can sign up for one of Eiki's fundraiser programs and work as a stall merchant on the Road of Liminality.
