Keine is still around, hasn't changed the slightest.
Reimu is dead, has a descendant.
Marisa may be either dead, or turned herself into a magician youkai.
Sakuya is still around, maybe looking "more mature" rather than "older".
Sanae is dead, certainly has a descendant.
Alice is still around.
Genji is still around, though probably not for much longer.
Rinnosuke hasn't changed one bit.
The Scarlet sisters still look like children.
The deva haven't changed.

In short, besides the miko, Marisa and Sakuya, not much has happened.
Come to think of it, the immortals have probably seen quite a few Hakurei miko come and go over the years.
