Depending on the size you can do the same as with a dakimakura.
Affix the onahole by wrapping a towel around it and the plush. You can secure the towel by either wrapping and tucking (like you would around your own waist) or with clothespins.
The best way would be to make your own insert, but that requires some sewing technique. You could buy some cheaper plushies to practice sewing on first.
