You are now in a coma, during which time you are in Gensokyo. You learn this after roughly two years; your real body has atrophied and your family, struggling with the burden of keeping you alive, is considering abandoning your treatment. If you remain in Gensokyo, your body will die, and you will be trapped in Gensokyo forever.

If you awaken, you will never be able to return to Paradise.
