There's 2 forms, the first one is to report slander, meaning falsely throwing shit at them.
The second form is to report content/videos that don't follow their guidelines, revealing personal information is one of them.
It's structured like:
>your name kek
>your phone
>your email (those last 2 are to contact you in case you have something more serious)
>content to report
Reports for something like this apply as "Privacy Issues" on YT and you get banned if you fake one. Youtube asks you for something to verify that in fact, the person in the video is you and you want it removed. Revealing their names alone is considered off-limits for Youtube.
