Monsters don't give much a damn about who you are one way or another, so long as not of the "unrepentantly evil" variety. Rich or poor is all the same to them.
However WG3 did mention that, given the choice, they tend to gravitate towards more "average" individuals as opposed to the cream of the crop, and I'd bet that's because they then get to influence that person's further growth as opposed to it being already done, more or less.

I imagine it stops at adulthood. That said, not every Kikimora will end up having them be the same length.
Some will end up with a relatively shallow heel, others might end up with 6+ inch ones. May depend on master's tastes.
