On the Extra stage, get precisely 666 graze points before reaching what would usually be Flandre, and you will also need to capture all of Patchouli's spells while moving in a pentagram motion around her. Then, granted you got your EoSD copy from under the truck, you will face Toutetsu instead of Flandre as a boss.
