Yes, sometimes the fantasy may be completely substituted in the imagination with a different one, but this doesn't remove the power of the context because it influences the performances and setup. If it's purely verbal as common in American porn, "you're my stepdad, this is wrong" or such, it doesn't have the energy of a performance like "this is wrong". Yes. PTJ-004 as a whole is highly surreal.
