Well it's less about artistic depiction of human body and more like about her third eye.
ZUN can't draw anatomy well so it's fair to take liberties even for accurate depiction, but it's kinda different with object. Even if it's still fall under artstyle limitation/choices, there's a point where it become too inacurrate from what is depicted and explained, like drawing Yuugi's horn at the size of a stick or drawing Kogasa with nylon umbrella, unless it's for stylization purposes.
The majority of Okuu's fanart significantly reduced the size of her third eye and is often depicted either as small pendant, cloth accessory or something tucked into her cleavage, even in supposedly regular depiction of her
Then again it depends on the nature and properties of her third eye, which I think has no detailed explanation yet, but I doubt it is something that can be altered or replaced to justify the radical changes.
