The silhouettes don't look like Mai and Satono and the game is supposed to take place before they went out and started empowering random people so it seems unlikely.

I've actually been thinking about Okina's powers a bit. At first glance, creating doors doesn't seem all that potent, but then I thought about it a bit deeper.
Consider, for example, Yukari's power. Manipulation of boundaries makes little sense, until you start to consider how boundaries are defined in Touhou. Boundaries, as we are told, represent the lines that make up everything in existence, the things that define form. As such, to manipulate an object's boundaries is to alter its very nature, and this is why Yukari is so potent.
So, if boundaries define things, what do doors do? Well, they serve as passageways, a link between things. So if Yukari can alter the form of something, Okina can alter the contents of something, and transfer things through barriers. This is why she's able to drain vitality and mentality through her doors. She is creating a passageway through which those things can escape you.

This is, of course, just speculation, but it's fun to think about all the same.

Despite being originally unimpressed with her design (because I wanted Yuuka), Okina has really grown on me. She just comes across as so cute, friendly, and yet powerful at the same time.

Not him and I disagree that Okina has a bad design, but I'm sure there was another way to represent her connection to the four seasons besides colored flames behind her, which Junko already did one game prior. It's kind of lazy of ZUN to do it again, even if the flames are a superficial similarity.
