Yasu's gender is never revealed, but all of the evidence points towards her being a girl.
First off, the entire plot point about Genji fearing that Kinzo might commit the "same sin again" and rape Yasu makes absolutely no sense if she was a boy. Kinzo is a very old-fashioned man with strict ideas about gender roles. He wouldn't somehow mistake a dick for a pussy and rape his son, just because he resembled his mother.
Genji and Kumasawa also wouldn't have raised an unsuspecting boy as a girl for no reason at all, while never telling him about his condition. It's just not in their character, especially not Kumasawa who grew quite fond of Yasu.
Last but not least, all of Yasu's problems (e.g not being able to menstruate, infertility or the lack of notable chest growth) are real-life aftereffects of a crushed womb. On the other hand, nothing indicates that a crushed dick would lead to smooth skin, feminine bodily and facial features or a female voice - which are all attributes Yasu possesses.
Kanon, Yasu's only male persona, has even been called out for appearing very feminine, whereas no one would ever mention anything of the sort in regards to Shannon.
