Rena is just a schizo that never fully got over her HS from her past when her parents got divorced. Even in the chapters where she's not the main HS victim, she's still fucking weird.
If she's following K1 around then she was worried about him and how he started to act differently upon learning some weird shit about Hinamizawa. His behavior mirrors a lot of what Satoshi did the year before leading up to his disappearance, which is the main reason Mion and Rena were so worried about K1. They didn't really deal with it the best way, but no one could know what Keiichi was actually going through in Oni.

Oni is definitely tragic from Mion and Rena's perspective. Their new friend starts showing the same signs that Satoshi did the year before when he went off the deep end and disappeared. They want to do all they can do avoid losing someone else in the same way. Unfortunately all of their attempts to continue the fun club days backfire and only push Keiichi further into paranoia, even the mentioning of him repeating Satoshi's behavior. There's no way for them to truly know what he's going through.
