The nutrients come from various sources
Synthesis within the cell (inside the mammary gland (carbs, protein etc.), the main driving force is the lactocyte
Synthesised from glucose inside the mammary gland (fatty acids)
Transported from plasma cells inside the mammary gland to the milk via receptors (vitamins, minerals, carbs, protein etc)
But of course the lactocytes need to get a source for all of these nutrients in the first place to produce milk. Basically this is done by having blood capillaries (whose purpose is to transfer oxygen and other nutrients around the body) surround the alveoli (which is lined by the milk producing lactocytes). The various substrates (minerals etc) are then diffused from the blood into the membranes of the lactocytes. The lactocytes then synthesise these substrates to produce that precious milk. This then explains why mothers actually burn calories when breastfeeding, because they're basically using up what they already have in the body to produce milk to feed the young, and can actually lose weight.
Since Letty hibernates this just raises a ton of questions as to how many calories she burns while breastfeeding during hibernation. In which case you could have a point as to Letty 'fattening' herself up so as to not lose a critical amount of weight every time she breastfeeds while hibernating and keep reserves up. Since bears actually feed their cubs while hibernating, then I don't see why Letty shouldn't produce breastmilk while hibernating either. In that case yes, chubby Letty would be a thing if she expected to constantly breastfeed fairies while she was asleep. But then we'd have to assume that Letty doesn't go into 'total hibernation' much like a bear, where it's better described as a 'torpor'.
While the amount of macronutrients don't necessarily change, what kind of acids and other components which make up these nutrients are in fact affected by the feeder's diet. Best I can say is that the nutrients which are the most variable and dependant on the mother's diet are the fatty acids such as omega-3 and omega-6 and micronutrients like Vitamin A, B1, and D.

Multiple reasons for sore nipples, but for simplicity we can assume that Letty's nipples would only feel sore if the feedees didn't latch onto them properly.

Breastfeeding produces the oxytocin hormone which facilitates social bonding and a 'closer connection' between the feeder and the feedee, so she might get a 'motherly' satisfaction from it.
