It's something you just are.
Look, hate to bring up ideas like "inner beauty" and things since very often that just isn't true. But true sexiness, not just an instantly arousing detail that provokes for the moment, is very much an overall inner quality. Not just looking, sounding or smelling good, but something you just have and radiate.

Come to think of it, for Keine there is a limit that isn't usually considered. She can only change history to what she can reasonably come up with - if there is a situation that is theoretically possible, she can't bring it about directly unless she has knowledge of it.
Certainly the Great and Wise Hakutaku is very knowledgeable, but she's still a single person with limited knowledge.
