I’m trying to draw a hand but I can’t get the pointer finger thumb area to look right. I usually draw cartoons and decided to try something more realistic. Any tips?

Why are you just trying to draw an outline instead of building up shapes? Why have you not looked at your own hand and looked at how Hogarth and bridgeman draw hands?

This. Proportions are especially important with hands. Get them right first and only then commit to lines. Details like nails and wrinkles come in last.

Cut off your hand and commission someone to do a taxidermy off it. That way yo always have a hand model you can view from any angle
