Also the art is REALLY important, no physics defying balloons, no retarded ahegaos (not all ahegaos are retarded though), no panels where you barely even understand what's happening from all the nonsensical goo, no half meter stomach bulges due to "the oniichan's nikubou is killing my insides".
Check Shiwasu no Okina or MARUTA for examples on how to make a good one (if you want ahegaos, hirame is probably the most exaggerated good one).
If you want some western inspirations, check out Alfie, it's a webcomic, the artist is really good with plots, always manages to keep it interesting. Keep in mind that some people can't stand exaggerated expressions if you have a more western style (Sunstone is the epitome of this, literally every panel has them biting their lower lip in the most unsexy way).

What I do: Watch an anime, normal series, read a book or w/e (or the series I'm drawing from). Then simply if something interesting gets my attention I go off from there. For instance, I saw this yandere character clinging to the MC who wasn't too happy about that; an anime. Got a plot about the mc being shitt tired of this bitch resulting in banging her brains out/sense...right.

To me, the most important aspects of doujins are believability and familiarity.

The whole reason rule 34 is popular is because it's seeing characters that the viewer already has familiarity or a connection with.
If you focus on that familiarity and put characters in situations that are at least somewhat believable for the characters and in the story then the reader will be more emotionally invested and will enjoy it all the more.

pic related is from one of my favorite doujins that illustrates this really well (Pretty Neighbor Vol. 6)
Basically it takes a scene that actually happened from the Yotsuba manga (Fuuka getting stuck in the window when trying to climb out and Koiwai helping her out) and adding a lewd twist.
Also the art style matches the source pretty well which is just bonus points.
