I find that mixing the known with the unknown is the easiest way to make things creepy. Juxtaposition and atmosphere are your friends.

Also, horror perfect place to get creative with form and mark making. Rough, loose marks can give an obsessive or violent impression. Exaggeration, stretched and warped forms, incongruous proportions, expressions and gestures, designs that look like they shouldn't be able to support their own weight.

Darkness may sound obvious, but doing a painting with a low-light, low contrast palette can make for a really disquieting image.
Generally speaking, the things that look scary look like predators, or the effects of disease. If you are not too squeemish, studying symptoms of deadly/debilitating diseases can give you good ideas for horror imagery.

And always keep in mind how important it is to have an element of the unknown in an image. An easy way to do this is to just not show everything, but you can also accomplish this by making something with an unknown purpose or function, or an image that is difficult to process visually.

If you want to design creepy characters like that, ambiguity and predator front facing eyes. Abiguity is why we find masks creepy, you cant read their facial emotions and work out their intention. Front facing starey eyes make us uneasy as they remind our evolutionary instincts of predators hunting us. Big drooling mouths full of teeth will do that too. Even just a few highlights around where the mouth might be could indicate salivating you can be subtle about it.

some of the genuinely creepier drawings/images I've seen combine essential, universal fears. >>4766006 for example combines fear of darkness (from water depth), fear of large things (face), ambiguity (face implies a giant body underneath, though it's not shown), general discomfort (eyes of abnormal proportions, facial expression), vulnerability (boat lads are probably fucked). you can break down any creepy image to the bare elements that make it creepy. likewise, you can build a drawing up from the core elements which would best evoke the types of fears you wish.
