In my experience, most people don't notice, most people who do are cool with it. If you ask, almost everyone will give you permission to draw them so long as they are somewhere it is normal to sit around for a few minutes anyway. I take my sketchbook to the gym and draw anatomy studies of the bros between my sets.

There used to be drink and draws at certain places in LA (15-20 dollars per session) where about 5 to 10% of the people are good.
