Up until very recently I couldn't understand the shapes of solid black on comic faces, or anywhere else, it all seemed half random.
Are they just inking the terminator and any surrounding shadow into a solid shape?

It isn't. You're talking about someone who had been drawing since he was 3, been majorly influenced by the very comics he would end up drawing for, and (funnily enough) was also heavily influenced by Andrew fucking Loomis. If anything, you should be saying

Alex Ross is skilled because rather than being content to read comics, he instead wanted to dissect them and recreate them in his vision.

You could probably "make it" too if you cared even half as much as he did in developing and refining your internal visualization of something you want to create.
