You're having trouble visualizing objects in perspective. This drawing shows you understand the goal of perspective but haven't practiced it enough to be able to see on your own when it's off because the features are somewhat skewed. It might help to practice drawing cubes in various rotations and drawing objects within those cubes. Also your head and facial structure understanding is a bit weak, doing some studies and checking out some head construction diagrams could help that.
