Let me just preface this by saying I'm not too good myself so this may be way off. Anyways I think you need to work on perspective. Remember that the mouth is also a 3d object that follows the rules of perspective, just study the shape and try to construct it like you would anything else. The eyes were also messed up. Kinda uneven and one is more sunken in than the other. Just try to keep perspective in mind.
