Sorry about the quality and size of the picture, but could I get some critique on this?

I know the pose is boring, and the guy was sort of a after thought, but shit still looks ugly to me. The nurse looks off and I feel like its barely above deviant art tier.... but I can't figure out why.

I've done 3 iterations of it and they all come out super plain and mediocre.
