Try to tell a story.

It doesn't even have to be a good story, that doesn't matters for raw visual art. Something to make it easier for you, like "This girl was scared of having sex at this location but she is still submissive enough to please her boyfriend." And then translate that to her body language.

Or you could try pic related.

always look awkward because the approach and penetration don't look natural (or comfortable) like they should be more perpendicular

I guess it depends on how much of the OC the commissioner wants to see so I guess you make have sacrifices
